“My Tool Box”, MUHC Chronic Disease Self-Management Program
The program is intended for adults and caregivers experiencing a wide range of chronic conditions including: Chronic Lung and Heart Diseases, Chronic pain, Cancer, Kidney Disease, Neurological conditions, Autoimmune disorders, Arthritis, Osteoporosis, and Diabetes. Workshops are scripted and highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.
New Hope Seniors Centre
New Hope Senior Citizens’ Centre is a nonprofit recreative centre serving adults 50+ in Montréal’s NDG since 1979. We provide a welcoming space with a Day Program, Meals on Wheels, and Special Programming.

Pastoral Home Care (SASMAD)
For more than twenty years the Archdiocese of Montreal has offered a service of spiritual accompaniment of sick and elderly persons in their own homes, under the name SASMAD, or as it is known in English: Pastoral Home Care. This diocesan service is confidential and free and is made available thanks to the visits of hundreds of volunteers who have received special training. Faithful to God who has called us, in the Church, to care for the sick at home, we make a commitment to respond to the spiritual needs of people who are suffering, within the context of the reforms in the healthcare system, whatever their beliefs.
